The Companion Animal Clinic is available for small animal internal medicine, small animal surgery and small animal neurology referrals.
Thank you for referring your patient to Midwestern University’s Companion Animal Clinic. If you are referring a patient to one of our specialty services such as surgery, internal medicine, or neurology, please complete the online referral and submit all medical records, including imaging, to our clinic email at: AHI@midwestern.edu. Medical records can also be faxed to 623-806-7401.
If your patient is critical or requires immediate attention, please contact our referral coordinator at 623-806-7465 or one of our front desk staff members at 623-806-7387. They will put you in contact with one of our clinical faculty members to ensure a smooth and seamless transfer.
Prior to sending a referral to the Companion Animal Clinic, please complete a referral form. If you prefer, you can print the form, and fax the completed version to 623-806-7401.
